Chronic Disease Management Plan referral (formally known as EPC referral)
We accept chronic Disease Management Plan referrals from your GP, under Medicare.
We subsidise our Medicare appointments to ensure Physiotherapy is more accessible. Gap Payment for Physiotherapy Initial Consultation is 50.40 (AUD) and 37.40 (AUD) for a Subsequent Consultation with a valid Chronic Disease Management (CDM) Referral from your GP
A valid Chronic Disease Management referral from your GP is required before making a booking
Our Physiotherapist have experience in treating DVA patients including white, Gold, and TPI card holders
A D904 referral from your GP is required to make a booking
